AUTHORIZATION AGREEMENT

By submitting this form, (I/we) accept a non-exclusive license to use the monitoring, chat, and click-to-call code features and other content comprising the WebRecept services for so long as WebRecept provides (me/us) the related services for which monthly fees have been collected. Upon a minimum fourteen (14) day period of unsuccessful monthly fee collection, (I/we) will no longer have the right to have our web site monitored or use the code, chat, click-to-call, or related services. This service agreement allows (me/us) to use all such related code, text, or other content in conjunction with (my/our) website and for customer service, marketing, and promotional purposes only. (I/We) understand that by discontinuing the recurring monthly payments to WebRecept, LLC, (I/we) are effectively forfeiting (my/our) rights for the ongoing use of their services on (my/our) website, and (my/our) WebRecept services may be removed from (my/our) website.
